Baby Piggy explains how she used to wear diapers. She used to get her diaper changed a lot every day, whether she liked it or not. She used to get diaper rash, and she used to get annoyed when her mother wouldn’t change it right away. One day she wanted to be like the big kids. So she decided that she’d had enough of wearing diapers. Now she goes on the potty; whenever she has to go, she says, I have to go now; she wears big-kid pants and she can pull them up all by herself. S

Manhasset, NY (Vocus/PRWEB) March 09, 2011

Having a child is a life event that is equal parts magic and fear. Will the baby be healthy? Will labor and delivery pose challenges? Are there ways to reduce the risk for adverse events? Hospital obstetrical units face these questions round-the-clock.

To increase the chances of a safe labor and delivery, and make way for a memorable birthing experience, the North Shore-LIJ Health System has launched a new prenatal quality initiative, led by Adiel Fleischer, MD, of obstetrics and gynecology at North Shore University Hospital and Long Island Jewish (LIJ) Medical Center, and Brian Wagner, MD, a maternal-fetal medicine specialist.

They designed and implemented a two-year comprehensive training program for all staff in the obstetrics wings at North Shore University Hospital and LIJ Medical Center. Staff was required to complete the formalized training that included evidence-based protocols to reduce adverse events.

These protocols included competency in reading electronic fetal heart rate monitors, communication skills and problem solving on a wide range of high-risk obstetrical emergencies through simulation programs. The hope was that the obstetrics initiative would lead to fewer adverse outcomes for mother and child.

It worked.

A team of scientists in the Division of Health Services Research in the health system’s Department of Population Health analyzed almost a dozen adverse outcome measures and found that such problems, including returning to the operating room and birth trauma, were significantly reduced by more than half – from two percent to 0.8 percent. Data also showed better outcomes were maintained over the two-year study period.

The analysis was published in the March issue of the Journal for Healthcare Quality .

In addition to improved outcomes, staff perceptions of safety improved significantly, as did perceptions among new mothers, who reported that their obstetrics teams were more cooperative and hard-working. What’s more, there were improvements in the documentation of abnormal fetal heart rate tracings and obstetric hemorrhage.

“The importance of the study and its long-term impact is that the safety measures that we introduced have provided better communication among the various healthcare providers. There is earlier identification of at-risk patients and team approach to patient care is critical to patient care,” said Dr. Fleischer.

Kenneth Abrams, MD, senior vice president of clinical operations and chief quality officer at the North Shore-LIJ Health System, agrees. “We improved patient safety and enhanced both staff and patient experience,” said Dr.Abrams. “This initiative shows that we can reduce adverse events and enhance a culture of safety.”

North Shore-LIJ hospitals deliver more than 21,000 babies every year, which represents nearly 10 percent of all births in New York State. Following the national trend to identify problems in the delivery of healthcare and reduce medical errors, clinicians implemented internal and external reviews of sentinel events, which included examinations of monthly obstetric charts. The chart reviewers looked at the documentation and management of obstetric hemorrhage and abnormal fetal heart rate tracings. The review led to a comprehensive list of potential causes of adverse events, including an inadequate escalation policy, and lack of standard protocol and standardizations pertaining to the interpretation for fetal heart rates. These factors are thought to play an important role in adverse outcomes.

As part of the program, Dr. Fleischer and his colleagues introduced multidisciplinary teaching rounds to help foster communication. During these daily meetings, the perinatal team reviewed and discussed appropriate assessment and management of obstetrical admissions. They enhanced the electronic medical record and implemented new protocols that have been shown to reduce the risk for adverse events.

Patients were contacted after they left the hospital and asked to complete a questionnaire about their experience during labor and delivery. Staff perceptions of safety were also assessed before and after the program.

“A critical component of the initiative was the educational process designed to improve recognition, appropriately document complications and avoid interventions that increase the risk of complications,” the researchers wrote in the journal. The study was led by Renee Pekmezaris, PhD, vice president for community health and health services research in the North Shore-LIJ Department of Population Health.

Dallas, TX (PRWEB) March 24, 2011

Author John Crawley announced the publication of his seventh novel, The Man On the Grassy Knoll.

The new novel is a work of historical fiction. With over 10 years of research into the countless theories about the assassination of President John F. Kennedy in Dallas on November 22, 1963 as a backdrop, Crawley introduces the reader to Raul Domingo Salazar. Salazar tells the story in his own words from his military prison cell outside Port-au-Prince, Haiti during intense interviews with a reporter named John.

The writing leads the reader into the prison interrogation room where John listens as Raul tells his story of being framed by the CIA and the Mob to keep him quiet about the role he played in the death of John Kennedy.

“This is by no means my longest or most ambitious novel,” Crawley explains. “It is, I think, my best. It is so terse and the story so direct, that one gets separated from the fact that it is fiction. It’s designed and paced to lead the reader into the prison interrogation room with the reporter to listen, first hand, to Raul Salazar as he describes being recruited to work first for the CIA and then for Castro. Later he describes in detail how he was trained for that day in Dallas and how the entire chain of events was planned and how it all went down.

“I wanted the experience to be as if you were reading a court transcript or the transcript of a tape recording made with Raul just days before. I wanted the reader to sit and listen to Raul tell his story, rather than reading a novel.”

The streets of Dallas in November in 1963 are recreated not in the author’s image and through his eyes, but rather through the memory of the second shooter that fateful day. Raul Salazar tells his story, a confession of sorts, about his life and that of the revolutions sweeping the globe in the ‘60s.

“There’s not one word in The Man On The Grassy Knoll, that isn’t from the experience of Raul, with the slight exceptions of the notes that the reporter adds to the manuscript to explain lapses on the tape or reactions to things that Raul has said. In so doing, the stripped-down story, the terse prose, is all part of the conversation we are being allowed to listen in on.”

As Crawley concludes, “Remember, this is fiction. Or is it?”

The Man On The Grassy Knoll is available at lulu.com and Amazon.com. Cover design by Bob Young of The Young Company, Dallas. Other novels by John Crawley include Baby Change Everything, Among the Aspen, The House Next Door, Under the Radar, The Uncivil War, and Between Sunday’s Columns.
Rockville, MD (PRWEB) May 20, 2011

Shady Grove Fertility Center is pleased to announce that its 2009 IVF Success Rates were recently published on the Society for Assisted Reproductive Technology’s (SART) website. Shady Grove Fertility exceeded the national averages in nearly every category for fresh IVF cycles.

According to SART, Shady Grove Fertility is the largest fertility center in the country based on the number of treatment cycles performed. In 2009, Shady Grove Fertility performed a total of 3,673 fresh IVF cycles, resulting in an average pregnancy rate across all age groups (under 35 to over 42) of 43% compared to the national average of 37%.

Eric Widra, MD, Board Certified Reproductive Endocrinologist at Shady Grove Fertility explains, “We know how important these success rates are to patients undergoing treatment and to those just beginning to explore their options for treatment. We also know that sorting through IVF statistics can be a challenge.”

Fertility center websites and other online forums present a wide array of statistics in many different formats. “For success rates to be a truly useful tool for patients, they must know who the trusted sources of data are, what the different reporting categories mean, and what the data reveals about individual fertility centers,” says Dr. Widra.

SART and the Centers for Disease Control (CDC) are considered the most trusted sources for IVF statistics, and their websites provide extensive information on assisted reproductive technology. According to the CDC, nearly all of the 436 fertility centers in the U.S., both SART and non-SART members, report their yearly data. Centers that do not report their data are listed on the CDC website as “nonreporters,” as required by law. Over 85% of all fertility centers are members of SART, and thus report to both organizations.

Some common questions fertility patients may have when reading these reports:

1. What are my chances of having a baby if I start a cycle? For this answer, patients would look at Live Births per Cycle, which may vary by age. For example, patients at Shady Grove Fertility age 35 and younger, have a live birth rate of 47.8% While live birth rates slowly decline as the patient’s age increases, a patient’s chance of success cumulates with each treatment cycle attempted.

2. What is the difference between Pregnancy Rate and Live Birth Rate? A pregnancy is determined after a positive pregnancy test (HCG) is received and confirmed by an ultrasound. The live birth rate is determined by the number of patients who deliver a baby. Because some pregnancies end in a miscarriage, the percentage of cycles resulting in pregnancies is higher than the percentage of cycles resulting in live births.

3. Does Implantation Rate Matter? This number demonstrates the quality of our Center’s clinical and laboratory work. A Center with good implantation rates should be able to transfer fewer embryos and still achieve high pregnancy success rates. If they don’t, this can be seen in the percentage of twin and triplet pregnancies. As desirable as a twin birth may sound after struggling with infertility, it is a riskier pregnancy.

4. What is the rate of Twins and Triplets Plus? Multiples pregnancies are higher risk and can result in higher rates of disabilities and other challenges for the child. Shady Grove Fertility is proud to have rates lower than the national average in both categories.

“While the numbers presented in our reports are critically important to us and to our patients, our success at providing high quality ethical and personalized care to our patients is equally important,” says Dr. Widra. “Our physicians, nurses and staff are dedicated to helping our patients achieve their goal of a healthy pregnancy and thus a healthy baby.”

5 English Speaking Rules : www.letstalk.co.in 3. Reading and Listening is NOT enough. Practice Speaking what you hear! Reading, listening, and speaking are the most important aspects of any language. The same is true for English. However, speaking is the only requirement to be fluent. It is normal for babies and children to learn speaking first, become fluent, then start reading, then writing. So the natural order is listening, speaking, reading, then writing.

From the innocence of Mady the squiggly ferret to the diabolical plans of the nefarious black sneaky cat, Crittertude is a place where life is magically transformed from the daily doldrum of existence to the realm of fantasy, imagination and the occasional unplanned mayhem. Fans of the online comic have felt a strong connection to each of the characters over the years as they have made us laugh, cry and ultimately realize how the simple unconditional love of a pet can transform our lives in ways never before imagined. The Crittertude book is available on Amazon at www.amazon.com Perhaps the rising star in all of this is Mady the squiggly ferret, who adds a certain unpredictability and air of innocence to the house. Booper the white fluffy dog is the guardian of the house and general underdog for the innocent (no pun intended). Lazy Orange Fat Cat is, essentially, a cranky behemoth with an appetite that is rivaled only by her girth. Melvin is our resident green cheek conure who constantly vies for attention and, according to a pet psychic reading once taken on a dare, secretly wishes that all the other animals in the house would just pack up, go away and die. Sitting in the cage nearby, our Zebra and Java Rice finches share a common nest and are the best of friends. Black Sneaky Cat is an easy-going, silent observer that is often undone by his own desire for playtime and overall power. Finally, there’s our dearly departed One of Six, the only surviving bird out of a litter …
